MCC USB-1608G activity light off

We have 4 DO probes wired into a MCC USB-1608G. The top green light is on, indicating the box is on. The bottom activity light goes on for a second after initially being turned on, but goes off. The DO probes all have readings of 0, which is not right. We've opened up InstaCal and did the analog test - the channels are reading <1V and not showing a square wave. I am struggling with interpreting what's going on and how to move forward to fix this.

Hello @jgmcdonough.

When testing digital output 0 channel on your USB-1608G with InstaCal's Analog Loop Back Test, if you do not see similar results as shown below from my test, then send me a private message to discuss repairing the device.  Include the device's serial number as shown in InstaCal and sales order information.

Both LEDs should be ON while the test is running.
