How to send in PCI-DAS6033 board for yearly calibration?

I cannot figure out how to send my PCI-DAS6033 board in for calibration.  Previously was calibrated under the MCC brand name.  Now with Digilent, how do I do this?

My company has several of these that are coming up due for calibration this month.
